HEALTHCARE PRODUCTS: POST INTERVENTION PROGRAM EVALUATIONS

Working with a large MCO in New England, the staff of Button Systems has performed a variety of post-implementation audits and outcomes analysis of chronic disease intervention programs, including: 

• Post hospital discharge follow-up of cardiovascular disease patients that encompassed PCP visit scheduling, beta blocker and ACE inhibitor prescribing, and specialist seen tracking.

• Cardiac Rehabilitation Outcomes program that tracked, over a three year period, all cardiac discharges in the state and how they fared in the three year period after participating (or not) in the various cardiac rehabilitation programs. Comprehensive analytical and graphical analysis done to evaluation effectiveness of the statewide rehabilitation program.

• Antidepressant Medication Follow-up program that reached out to the patient level and prescribing provider in an effort to improve the effectiveness and Rx persistence of the program.

• Immunization Follow-up Program that interfaced with pediatricians and GP office staff in an effort to improve the reporting and issuance of childhood immunizations.

• Mental Health Discharge Follow-up program that tracked and reported the progress of individuals discharged from the State’s 14 hospitals when diagnosed with mental health issues, tracking Rx, visit frequency, and specialist access over time.
